CHAP Regional Resources
The Cardiovascular Health Awareness Program (CHAP) is a community program to improve heart health among seniors. By partnering with local family medicine providers, community volunteers and organizations, CHAP aims to provide seniors with information about community programs and resources that can help to adopt and maintain healthy lifestyle practices.
This site is intended for seniors in downtown Ottawa who have attended a CHAP session at their family medicine office, or who are a looking for heart health information and programs available in Ottawa. Included on this site is information on healthy eating, physical activity, quitting smoking, and living well with diabetes, high blood pressure or heart disease.

Living Well with Heart Disease and Stroke
For more information on preventing and living with high blood pressure, heart disease and stroke, visit Hypertension Canada and the Heart and Stroke Foundation of Canada.

A number of programs are available in your community to help you improve and maintain your health while living with heart disease and stroke. Many pharmacies also provide blood pressure checks and have self-service blood pressure kiosks.

The CHAP is an evidence-based cardiovascular health awareness program for seniors *. In a previous study involving 39 small to mid-sized Ontario communities, the Cardiovascular Health Awareness Program (CHAP) resulted in significantly fewer hospitalizations related to heart disease among seniors. (1) Seniors from select family medicine practices n downtown Ottawa will participate in CHAP sessions beginning in Fall 2014.

The Cardiovascular Health Awareness Program (CHAP) is a community-based program that aims to promote a healthy lifestyle and prevent high blood pressure, diabetes and heart disease among seniors.
