Programs Description
Community Kitchen for Seniors Cook, eat, and learn new recipes to help manage diabetes, high cholesterol, or high blood pressure. Group is facilitated by a dietician who specializes in chronic disease management.
Smoking Cessation Program for anyone who wants to quit. Includes counseling, free nicotine replacement therapy for registered client.
Seniors Shopping Groups Weekly Monday morning transport for 60+ to local grocery stores and shopping malls.
Chinese Seniors Exercise Program Exercise and social activities for Mandarin and Cantonese speaking seniors
Finding Food in our Neighborhood Food map to minimize the effects of the "Food Desert" in the area.
Yet Keen Seniors day Centre Program for Chinese seniors to promote physical and mental well-being: includes dance classes, recreational activities, fitness classes and more
Social Service Walk-In Social services and urgent assistance provided for immediate help with personal crisis, food security, etc.
Physical Activity Programs Include exercise drop-ins, fitness assessments, group exercise, individual programs

Living Well with COPD Program to help learning the management of COPD through exercise and self-management skills
Asthma Education Assessment, evaluation and education on managing asthma with a Certified Asthma Educator.
Counselling Individual, short-term services for adults and seniors. Help with a wide range of issues including mental health, relationships, domestic violence, abuse. Services available in English, Mandarin, Cantonese, Vietnamese and Cambodian. Also offers groups that focus on anxiety and depression.

Walk-in  Provides a full range of health services to clients of SWCHC, residents of the service area, and people who do not have an Ontario health card or permanent address. Nurse Practitioners provide assessment, treatment and referral if required. Open Mondays and Fridays, 10am-4pm, Tuesday and Wednesdays, 10:30am-7pm, Thursdays 1pm-7:30pm (from September to June) or 10am-7:30pm (July and August).
Foot Care Foot and nail care
Acupuncture Available by appointment Tuesday, Wednesday and Thursday.
Free Dental Screening Call for times and places.
Dietitian Consultations Registered dietitian provides individual and group counselling.
