Program that grants wishes to youth with life-threatening medical conditions

Application:

A child may be referred by a medical professional (doctor, nurse, social worker, child life specialist, case worker etc) with the permission and consent of the child’s parents or a child can be referred by anyone including his or her own parent, relatives, friends etc (consent is required by the parent or legal guardian)
