Service Description: Using a Health Links approach, registered nurses and community health workers visit vulnerable seniors in their homes to provide support with their care and link them to appropriate community resources to assist them with their various needs * Program works in collaboration with health professionals and other agencies to reduce isolation and support high risk seniors living at home

Eligibility / Target Population: Seniors 65 years and older who may be isolated and at risk of hospitalization
Languages: English ; French ; American Sign Language (ASL) ; Arabic ; Chinese (Cantonese) ; Chinese (Mandarin) ; Italian ; Somali ; Spanish
