Service Description:
|
Mental health treatment, rehabilitation and support services for identified clients with severe and persistent mental illness
Services:
- crisis intervention (clients of ACT only)
- education, support and consultation to clients' families
- medication prescription, administration, monitoring
- social, interpersonal relationship, and leisure-time skill training
- substance use counselling
- support with activities of daily living
- symptom assessment, management, individual supportive therapy
- work related services

Referral to the Carlington Assertive Community Treatment Team are done through the ACTT Central Intake Office at 613-722-6521 ext 7325

Eligibility / Target Population:
|
Persons 16 years old and over with persistent or chronic mental illness
Persons with a high service use as defined by being in an institution on the basis of a mental illness for more than 90 days continuous, 50 days in the past year or 150 days over the past 3 years. Additional consideration is given to clients who exhibit difficulty with homelessness and incarceration
Persons with a significant functional impairment demonstrated by: the inability to maintain activities of daily living, the inability to maintain personal safety, and the inability to participate in existing services
Complexity of presenting problems given priority, such as coexisting substance use of more than 6 months, co-morbidity of psychiatric diagnosis, or criminal justice involvement
