Service Description: The Community Wellness Worker (CWW) Program: supports holistic approaches to the promotion of healthy living and the prevention of family violence * Community Wellness Workers in communities across Ontario put the needs of local Métis communities first through the implementation of client driven services to reduce family violence and the promotion of healthy lifestyles through culture based programming * Community Wellness Workers also provide participant based services with regards to reducing family violence through fitness and recreation, walking groups, nutrition and healthy eating workshops * Community Wellness Workers also promote information sharing between Indigenous and non-Indigenous organizations including mental health centres, shelters, and treatment centres to increase client access to local services

The Metis Healthy Babies Healthy Children (MHBHC) Program: designed to help children and families from the pre-natal stage to six years of age * The goal of the Program is to ensure equitable access to resources, support, and knowledge for Métis families so that their children have the best possible opportunity to thrive * The Program offers preparation for parenting,  pre-natal and post-natal care * The goal is to assist all Indigenous families in providing an environment for the healthy development of children through home visits, service coordination, and referrals * The Program which provides support for children at risk is voluntary and open to any Indigenous family who requests the service

The Aging at Home (AAH) Program: offers support to Indigenous seniors 55 years of age and older by offering in home supports and contracted services to help individuals live safely and independently within their own homes * There are some eligibility criteria that a person must meet in order to be eligible for the Program * This includes having a financial review done and having a medical referral

Services available through this Program include:
  • In home cleaning
  • Transportation for running errands
  • Contracted services such as lawn care and snow removal

Video conferencing suites are available on-site and use Ontario Telemedicine Network (OTN) to provide psychiatry and other health care specialists with a remote connection to patients and clients

Application: No referral required
Eligibility / Target Population: People who self-identify as Métis, are not enrolled on any other Aboriginal registry, and are of historic Métis ancestry or reside in Métis communities
