Service Description:
|
Tele-cardiopulmonary rehab services for cardiac and pulmonary exercise class participants, in collaboration between the University of Ottawa Heart Institute, St Francis Memorial Hospital and the Lanark Renfrew Lung Program:
- ongoing clinical cardiac rehab training
- connects via Ontario Telehealth Network to OHI for pre-designated exercise sessions
- education and resources for patients
- cardiac rehabilitation nurses report, monitor, and evaluate participants’ progress
- development of an individualized plan
- self-management counselling
- risk factor reduction
- cardio-pulmonary conditioning
- follow-up and collaboration with other care team members
