Service Description:
|
Primary Care Medical clinic that provides comprehensive team based health care services * primary care services are offered through Nurse Practitioners in collaboration with an inter-disciplinary health care team of Registered Nurses, Community Mental Health Workers, Dietitians and Physiotherapist
All services are free * services include:
- conducting detailed health assessment to make a diagnosis
- treatment plans and medication recommendations/prescriptions
- referrals to medical specialists
- blood work collected on site
- childhood and adult immunizations
- chronic disease management, support and prevention counselling
- well baby and well-mom visits
- prenatal primary care
- nutritional counselling
- mental health services and supports
- spirometry services
- smoke cessation supports and counselling
- specialized hypertension services
- CardioPrevent services
- intensive care coordination and support with system navigation is also offered
Partnerships include:
- CardioPREVENT program in partnership with the Ottawa University and Heart Institute
- Low Back Pain Chiropractic services as part of the Provincial Low Back Pain Program
- complex diabetes clinic in partnership with the Cornwall Community Hospital
- timely access to psychiatric services and consultation
