Call out for Patient Advisors
The Champlain LHIN is seeking patients and family members to participate in the Patient and Family Advisory Committee (PFAC). This committee will assist the LHIN in shaping its programs, services and initiatives to improve health care for people in our region.
To apply to become a member of the Champlain LHIN Patient and Family Advisory Committee, the interested person must submit this Expression of Interest form, and a current CV, if applicable, to lise.racicot@lhins.on.ca or by mail to:
