Ontario Launching Online Program to Help Individuals Manage their Chronic Conditions
Adults across Ontario living with chronic conditions can soon participate in a free Online Chronic Disease Self-Management Workshop to take control of their health. Broadly accessible from any computer with an internet connection, the workshop provides the skills and confidence needed so that participants can better manage their chronic conditions and live healthier lives.
To be launched in February 2016, each online workshop is led by two peer facilitators who are often living with chronic conditions of their own. Sessions are highly interactive and provide mutual support, with groups of about 25 participating together. Participants log on at their convenience during the 6-week workshop for a total of about 2 hours per week.
Content is consistent with regional in-person workshops across Ontario, and now individuals can participate confidentially and anonymously on a secure and dedicated website. Caregivers, friends, and family members of someone with a chronic health condition are also welcome to participate. This evidence-based online program was designed at Stanford University and made available to Ontarians through funding by the Ontario Ministry of Health and Long-Term Care.
