CardioPrevent Mobile Clinic
Novel initiative at the Heart Institute with the support from the Cornwall Community Hospital, Eastern Ontario Health Unit, Heart & Stroke Foundation of Ontario, Hôpital Glengarry Memorial Hospital, Glengarry Nurse Practitioner-Led Clinic, Riverdale Terrace Waterfront Retirement Residence and Seaway Valley Community Health Centre.
Together we are bringing the CardioPrevent Program to Stormont, Dundas and Glengarry Counties in October. This innovative, evidence-based program aims to lower the risk of cardiovascular disease (CVD) in those who do not yet have established disease, but have been identified at a moderate to high risk.
Over a one year period, CardioPrevent staff will guide participants through a customized lifestyle program based on their personal risk factor profile. The Heart Institute will work with referring primary care providers to engage and support sustained heart healthy behaviour change in their patients.
The CardioPrevent Program is free of charge and accepts individuals who are at a moderate to high risk for CVD. Program entry requires a primary care provider referral.
