Programs Description
Cardiovascular Chronic Disease Management Program The team will assess your cardiovascular risk and help you optimize your health through education, prevention, and treatment. For patients with high blood pressure or cholesterol.
Craving Change Program How-to workshop by registered dietician and mental health educator to understand and improve your eating habits.
Dietitian Services Counselling from a registered dietitian, who can provide you with advice about your diet and help you make healthy food choices.
Healthy Lifestyles Program A lifestyle approach to help you reach and maintain a healthy weight. The program is offered in a group setting to enable you to share experiences and offer each other support. Sessions are co-facilitated by our registered dietitian and a certified fitness trainer.
Smoking Cessation Program Provides strategies to help you to quit smoking and follow-up counseling once you leave the clinic to help stay smoke-free.
Medical Review with Clinical Pharmacist One-on-one consultation with a pharmacist to review your medications, answer questions and maintain a list of your current medications.
Foot Care with UOHS Chiropodist Assessment by our chiropodist to help you achieve optimal foot health.
Diabetes Program Focused diabetes care available to support you. Includes registered dietician, chiropodist, smoking cessation counselor, and counseling team.
Walk-in Clinic Open 7 days a week to everyone, including Quebec residents. *Note: the Walk-in Clinic is closed on Sundays from June to August, on December 25th-26th and on January 1st.
