Programs Description
Community and Home Support Services Services for seniors age 60+ living in the South East Ottawa service area, which include transport to medical appointments, grocery shopping, and support in the home after hospital discharge.
Primary Care Outreach to Frail Seniors (PCO) Nurses available to visit frail seniors in home to support them with care plan
Urban Pole Walking Some of the benefits of Nordic walking include strength, posture and conditioning benefits while enjoying a social setting. Requires registration at the Community Health Centre to assess fitness readiness.
Viactive Exercises for Seniors Exercises include chair exercises, strength exercises, dance movements.
Requires registration at the Community Health Centre to assess fitness readiness.
Smoking Cessation Offers individual counselling and Nicotine Replacement Therapy, with a Nurse Practitioner on an ongoing basis
Craving Change – Weight Management Support Program Learn how to change your relationship with food.
From Soup to Tomatoes Exercise program provided by the Ontario TeleMedicine Network. Geared towards adults and seniors, a kinesiologist and registered dietician lead exercise classes via video conference. Requires registration at the Community Health Centre to assess fitness readiness.
Social Service Walk-In Social services provided to those who need help with a personal crisis, food security, referrals, and community resources. No appointment required.
