Reports To: Care Coordinator / Director of Care
Position Summary
The Personal Support Worker (PSW) provides high-quality personal care and assistance with daily living activities in clients’ homes or community settings. The PSW plays a vital role in ensuring clients feel safe, respected, and supported.
Key Responsibilities
• Provide personal care (bathing, grooming, dressing, toileting)
• Assist with mobility and safe transfers
• Support meal preparation and feeding assistance
• Perform light housekeeping related to client care
• Provide companionship and emotional support
• Monitor and report changes in client condition to the Care Coordinator
• Assist with medication reminders (as per care plan)
• Maintain accurate documentation and client records
• Follow infection prevention and safety protocols
• Uphold client confidentiality and professional standards
Qualifications
• PSW Certificate from an accredited institution
• Current CPR/First Aid Certification
• Up-to-date immunizations (including TB screening)
• Vulnerable Sector Police Check
• Previous home care or long-term care experience preferred
• Valid driver’s license and reliable transportation (preferred)
