Service Description: Coordinates, promotes and facilitates self-management programs across the Champlain Local Health Integration (LHIN) network * self-management programs help to: cope with disease and live better quality lives * develop self-efficacy and self-confidence * manage their own chronic conditions and interactions with the health care system and individual providers * provides a centralized program to improve coordination of and access to self-management supports, programs and resources for people with chronic conditions and chronic pain

Chronic conditions include diabetes * kidney disease * arthritis * heart disease * respiratory disease * chronic pain * asthma * others

Application: Supports several different workshops throughout Ottawa and surrounding regions for people with chronic conditions and chronic pain and/or their caregivers * for information on workshop dates and times, visit website or call the toll free number
