Service Description:
|
A publicly funded, community-based health care service that provides eligible seniors with the support and services they need to allow them to continue living on their own in their homes for as long as possible
Assisted Living Services can provide non-medical services that include:
- Personal Support Services (personal hygiene, activities of daily living)
- Homemaking Services (light housekeeping, meal prep, making shopping list and laundry)
- Safety and Reassurance checks (via phone or in-person)
- 24 hour urgent response
Staff work with clients to determine the best mix of services to meet their need, and then preschedule the services at the times of the day when the clients need them * Services are available every day, 365 days of the year * Clients are given electronic response systems to allow them immediate telephone access to ALS staff in the event of an urgent situation

Eligibility / Target Population:
|
Adults aged 65 years or older * Live in private home or apartment building, within 15 minute drive of Renfrew Victoria Hospital, Calabogie Medical Centre or Cobden Connect Well Community Health * Demonstrates a need for daily access to personal support services throughout a 24 hour period * Able to direct their own care or have a substitute decision maker or live-in caregiver to direct care * Can communicate their needs (with or without aides) * Are medically stable (medical/professional needs can be met by HCC_SCC, family physician or other community providers) * Pose no risk to themselves or others
