Service Description:
|
Provides care to patients who perform their dialysis treatment at home within the Champlain region * Peritoneal Dialysis (PD) and Home Hemodialysis (HHD) supported * supports patients and their families to self-manage their renal illness, including the dialysis treatments, while promoting the development of personal skills for health and wellness
Provides training and education to ensure safe and effective home dialysis * early identification and treatment of related problems thus ensuring patient safety, satisfaction and retention on the home dialysis program * engages patients and families in identifying and actioning their goals of care as circumstances and response to illness change
