Service Description: Intensive mental health case management services for youth & adults * limited permanent affordable housing options
  • Mental health family focus case management program offers mental health supports based on the strengths based model, with the individual as primary client * services are extended to encompass the nuclear family where appropriate and approved by the primary client
  • AMANI (Mental Health and Substance-Use Health Program for African Canadian and Caribbean Youth): culturally adaptive Mental Health and Substance Use Health support to Black youth (12-29yrs)

Application: Community referrals from mental health professionals, doctors, social workers, self or family for mental health case management * individuals should apply through the Mental Health Community Support Services (MHCSS) Central Intake portal housed at the Canadian Mental Health Association - Ottawa Branch at 613-737-7791

Psychiatric Assessment clinic referrals must come through a recognized partner agency such as a Community Health Centre, Mental Health Community Support Services or Ottawa Youth Justice Service Network partner agency
Eligibility / Target Population: Youth aged 16-25 and Adults 25 years and older with a severe and persistent mental illness in Intensive Case Management program * SAPACCY youth ages 12-29 yrs
