Service Description: Rehabilitation and healthcare services for people with low vision * supports clients in maintaining safety and independence * assistance with:
  • choosing low-vision aids and assistive devices for daily tasks
  • learning safe methods for meal preparation and using household appliances
  • techniques for organizing and identifying household items
  • navigating indoor environments
  • falls prevention
Specialized Services: Supports for clients to achieve long-term, advanced goals * specialists may help with developing essential skills for daily living, travelling independently, and using assistive devices

Early Intervention Services: Supports for children with low vision and their families * early interventions help families understand child’s vision loss, address concerns, identify and refer to specialized services, and transition into community activities, child care, and school

Fees: Fees associated with assessments done by the Regional Assessment Centre for the Assistive Devices Program (ADP) * fees for service for worksite assessments, school based assessments and treatment or other requests for private businesses * None for OHIP funded rehabilitation services, including assessments and treatments for all eligible clients
Application: Referrals accepted from health care professionals for clients experiencing functional difficulties in their daily life related to vision loss * Referrals can be submitted online at on.visionlossrehab.ca * may self-refer but asked to have eye care professional provide recent information on vision history
Eligibility / Target Population: Residents of Eastern Ontario of all ages living with vision loss and Deaf-blind adults
Languages: English * French * American Sign Language (ASL)

Frontenac, Hastings, Lanark, Leeds and Grenville, Renfrew, and Stormont, Dundas and Glengarry
