Service Description:

Support to adults with developmental disabilities living in their own homes or apartments * Programs are based on Person Centered Planning and delivered usually in the home
Services can include:
- meal preparation
- banking
- house keeping
- medical appointment
The level of support is determined on an individual basis and can range from several hours per day to a few hours each week.
