Service Description:
|
Integrated community mental health support services for individuals with a severe and persistent mental illness who are homeless or at risk of homelessness with complex needs
Services include:- outreach (outreach, court, hospital)
- case management
- housing
- concurrent disorders treatment program
- health promotion
- Training and community development to respond to the needs of individuals with serious mental illness, education, training and distribution of materials

- Integrated Treatment for Concurrent Disorders
- Mental Health Works (for employers and employees in the workplace)
- Living Life to the Full
- Motivational Interviewing
