Application: During the first visit, personal information will be entered on a client form, including client name, birth date, address, telephone number, spouse/partner name and birth date, number of adults and children in the family, children's birth dates, accommodation (rental, own, etc.), sources of income (CPP, ODSP, EI, OW, employer name, etc). Documentation required includes:Identification: includes Birth Certificate, Health Card, Driver's License, etc.Proof of Address: any formal document that has recently been mailed to you at your current address and that shows the date, your name and your address. Examples of a Proof of Address include a bank statement, utility bill, a letter from a government agency, etc.School-age Children: school report card for each child? If the documents cannot be provided on the first visit, the client can still obtain food, but the documentation must be provided on the second visit? Documentation is required twice a year, to confirm that the information on your form is correct and up-to-date
