Application:

During the first visit, personal information will be obtained, including client name, birth date, address, telephone number, family members' names and birth date * Documentation required includes: Photo ID for client, and ID for each person in the family * Proof of Address: any formal document (paper or electronic) that has been received by the client within the past 3 months that shows the date, name and current address * Examples of a Proof of Address include a bank statement, utility bill, a letter from a government agency, etc * Client Photo ID and Proof of Address are required for each visit
