Service Description:

Satellite diabetes management education program
Registered nurse and a dietician in partnership with the client's physician * participants learn to better control their diabetes and help prevent health-related complications * program consists of an initial evaluation, a group session and individual follow-up * manages pre-diabetes and type 2 diabetes
