Service Description: Supports for people with Alzheimer's disease and other dementias, and their caregivers

A Dementia Care Coach provides expert support to improve the quality of life of caregivers, families and people living with dementia * works with people and families impacted by dementia in all its forms

Provides supportive individual and family coaching to persons with dementia and their caregivers, to support self-management goals and strengthen linkages between providers across the continuum of care

Seeks to recognize and respond individually to clients' changing needs through:
  • Individualized care coaching
  • Education, support and referral to specialized geriatric programs
  • Navigation of the care system and referral to other community support services
  • Strategies for self-management
  • Assistance in reducing social isolation by connecting people to social programs

"About Us"
The Dementia Society receives funds from Ontario Health and through additional fund development initiatives, we develop, deliver and innovate a wide range of social, recreational and educational programs to reduce isolation and engage people living with dementia and their caregivers and families in meaningful and supportive activities so that no-one faces dementia alone.
