Service Description: Health centre focused on family and community health, including:

Primary Care Walk-in Clinic available on Wednesdays from 1pm-8pm for rostered clients of the practice, anyone living in the Somerset West Community Health Centre service area who does not have a primary care provider, including those without an Ontario health card and/or a permanent address

Primary health care operates by appointment at the 30 Rosemount Branch * Harm reduction services are offered at the 55 Eccles St location only

Fees: None * accepts patients without coverage from the Ontario Health Insurance Plan (OHIP) or money to pay for health services
Application: No referral required
Eligibility / Target Population: For most programs: People must live within the service area
