Service Description: Cultural and medical supports for the Indigenous community at risk of developing diabetes, or living with prediabetes or type 2 diabetes * Supports and services offered in a culturally-sensitive environment
  • traditional seasonal healthy living activities
  • insulin initiation and management
  • nutrition and healthy eating
  • physical activity (group fitness)
  • family support and education
  • weekly group education classes and health workshops

Application: Self-referral or through Diabetes Central Ottawa
Eligibility / Target Population: First Nation, Inuit and Metis peoples 18 years and older at risk of developing diabetes, with diabetes or pre-diabetes
