Interdisciplinary team focused on primary care for its rostered patient population
Full range of primary health care services including preventative screening, dietitian services, nursing services, prenatal care, post-partum care, well-baby checks, sexual health services, counselling, etc
Team includes physicians, nurse practitioners, registered practical nurses, dietitian, social worker, and administrative support staff who work collaboratively to deliver person-centered care
