Service Description:	Meals on wheels (soup/salad, entrée, dessert/fruit) * delivered Mon, Wed, Fri starting 11am supplementary meals therapeutic diets textures available as needed service assessment (screener) will be completed upon service request and annually thereafter service delivery provided by volunteers
