Access to support at home or in the community
- respite
- personal care such as bathing, grooming, dressing and exercise programs
- meal planning and preparation
- companionship, motivational activities and socialization
- household management such as laundry and light housekeeping

Application:

Self or health professional referral * agency and client will determine the schedule based on a maximum of 15 hours per week
|
