Launch of the 2018 Minister’s Medal Honouring Excellence in Health Quality and Safety

The Ministry of Health and Long-Term Care is pleased to announce that it is now accepting applications for the 2018 Minister’s Medal Honouring Excellence in Health Quality and Safety. This annual, competitive recognition program aims to honour exceptional work and showcases successes across the health care system.

The 2018 Minister’s Medal theme is Innovating Integration, which recognizes the creative and innovative initiatives being undertaken across the province to foster an integrated health system in order to achieve better patient and provider experiences, better health outcomes, and better value in health care delivery. This theme also recognizes health champions focused on strengthening partnerships and integration to ensure a seamless patient journey.

The LHINs have been a key partner in the success of the Minister’s Medal over the last five years, and I am writing to ask for your support again this year. Not only do the LHINs play a key role in promoting the program and encouraging local health partners to apply, but also in supporting the application review process by receiving and evaluating applications in advance of the final external advisory committee review.

Application templates are now available on the ministry website at:

The deadline for submission to the LHIN is 5:00pm on August 24, 2018

Winners will be announced at Health Quality Ontario’s conference, Health Quality Transformation 2018, on October 17, 2018.
