Ministry of Health and Long-Term Care Survey
The Ministry of Health and Long-Term Care is conducting a review of a government-funded program, ConnexOntario Health Services Information. This program review will examine the services provided by ConnexOntario and identify opportunities to make it easier for Ontarians to be connected to mental health and addictions services.
The purpose of this survey is to understand how you find (or would find) information about mental health, addictions, and gambling services. The survey will take no more than 10 minutes to complete.
