The Ontario Medal for Good Citizenship

Established in 1073, the Ontario Medal for Good Citizenship honours who, through exceptional, long-term efforts, have made outstanding contributions to community life.

Recipients will be presented wit their medal by the Lieutenant Governor of Ontario at a special ceremony at Queen's Park in the Fall of 2015.

Here is what you need to do to submit a nomination for this medal program:
  • Visit ontario.ca/honoursandawards and click on the Ontario Medal for Good Citizenship icon.
  • Download the appropriate PDF form.
  • Read the eligibility criteria and instructions carefully.
  • Fill out the form and submit it along with your supporting documents/testimonials before July 17, 2015. Instructions for submitting your package can be found on the website
