Request for Service Proposal – Diabetes Foot Care Toolbox Workshop Facilitator

Centretown Community Health Centre, on behalf of the Champlain Diabetes Foot Care Expert Committee, is currently accepting proposals from skilled facilitators to offer Diabetes Foot Care Toolbox workshops to health care professionals across Champlain. Specifically, this request is for a Diabetes Foot Care Toolbox Workshop Facilitator to deliver workshops with a standardized curriculum. The delivery of these workshops will be supported by the Champlain Diabetes Foot Care Expert Committee and the Champlain Diabetes Chiropody Program.

The objectives of these workshops are to:

  • Disseminate and promote uptake of evidence-based foot care best practices for diabetes
  • Promote and increase awareness of the need for regular foot examination and ulcer prevention strategies for people living with diabetes
  • Demonstrate how to perform an evidence-based, standardized diabetes foot risk assessment, stratify risk and identify when to refer for treatment
  • Identify how and where to refer patients to treat and prevent reoccurrence of diabetic foot ulcers
  • Discuss ways to provide clients/patients with appropriate education about diabetes foot care

These workshops are a priority activity to build capacity among health care providers and advance the Champlain Diabetes Foot Ulcer and Amputation Reduction Strategy.

Background
Diabetic foot ulcers are a common, costly, and burdensome complication of diabetes. They precede 85% of lower limb amputations and are largely preventable with risk assessment, early referral and patient self-management education. Foot ulcer and lower extremity amputation risk reduction has been identified as a priority focus by the Champlain Regional Diabetes Advisory Committee in response to concerning reports indicating that the Champlain LHIN region has a disproportionally high incidence of lower extremity amputations and low foot examination rates. As a result, the Champlain Diabetes Foot Care Expert Committee was established in 2011 to develop and implement a Diabetes Foot Ulcer and Amputation Reduction Strategy.

Phase one of this strategy focused on diabetes foot ulcer prevention. During this phase, the Expert Committee developed a best practice “tool box”, education workshops, and evaluation plan to support the uptake of best practices for reducing foot ulcers and amputations for people living with diabetes across the Champlain region. This toolbox is currently available at www.champlaindrcc.ca in both English and French and includes documents and resources for health care providers and patients. To date, 157 individuals have accessed the online toolbox and over 110 primary care providers, community nurses, diabetes educators and other allied health professionals have participated in the corresponding workshops.

Two Diabetes Foot Care Toolbox Workshops have already been developed and tested: one for community-based nurses (approx. 2hr session) and one for primary care providers (approx. 1hr session). The Diabetes Foot Care Workshop Facilitator will be responsible for delivering these workshops across Champlain.

Project Scope

  • Identify workshop locations and support the recruitment of participants. This work will be done in close liaison with the Champlain Diabetes Foot Care Expert Committee.
  • Offer a minimum of 6 Diabetes Foot Care Toolbox Workshops. These workshops will be targeted to both community-based nurses and primary care providers.
  • Develop and implement at least one Diabetes Foot Care Toolbox Webinar
  • Submit the workshop and webinar for MainPro certification to increase incentive for physicians to participate.
  • Draft a final report summarizing the total number of workshops, participants and the feedback from evaluation forms.
  • All work is to be completed by March 31 2015.

Budget
All proposals must include proposed costs for salary (up to a maximum of $9,555) and mileage (up to a maximum of $2000) to complete the tasks described in the project scope. All workshop supplies, promotional materials, and technology to develop and host a webinar will be provided in addition to this budget; it is not expected these expense be identified in the proposal.

Bidder qualifications
  • Registered Nurse, Chiropodist or other qualified health care professional
  • Strong teaching and facilitation skills
  • Strong verbal and written communication skills. English required. Bilingual preferred.
  • Experience providing both preventative foot care and treatment for individuals with diabetes
  • Strong computer literacy, including MS Office (essential) and Adobe Connect (preferred)
  • Access to a home office with computer, internet and telephone
  • Access to a vehicle and valid driver’s license to travel across the Champlain LHIN region

Proposal submission guidelines

All proposals should include:
  • Cover letter including description of experience facilitating interactive workshops and providing foot care for people with diabetes
  • Resume
  • A project outline that identifies your approach to complete the deliverables, proposed timeline and budget

Proposal evaluation criteria
The Champlain Diabetes Foot Care Expert Committee will review all proposals based on the following criteria:
  • Previous experience facilitating group workshops with health care professionals
  • Previous clinical experience providing foot care for individuals with diabetes
  • Previous experience liaising with community stakeholders, especially health care professionals and administrators
  • Ability to complete project by March 31 2015 within budget
  • Overall suitability to work with the Champlain Diabetes Foot Care Expert Committee, the Champlain Diabetes Chiropody Program and multiple community stakeholders
