New Assisted-Living Program for Pembroke Seniors



Services to help seniors stay healthy and independent at home


Seniors in Pembroke will soon benefit from a new program that will help them stay in their own homes longer. The program, funded by the Champlain Local Health Integration Network (LHIN) and operated by the agency Carefor Health & Community Services, will provide 20 new assisted-living spaces in Pembroke. Clients will receive homemaking, help with personal hygiene, care coordination, security checks, and a 24/7 on-call service – all in their current place of residence.

The new assisted-living program has many benefits. It will expand seniors’ services in Renfrew County, relieve pressures on hospital emergency rooms, and prevent premature admissions to long-term care homes, freeing these beds for people who need them the most.

After consulting with Pembroke-area providers about the needs of seniors, the LHIN determined that an assisted-living program was the best program option in terms of effectiveness and viability.

The Champlain LHIN is providing $397,000 on an annual basis to Carefor to deliver the new services. Other partners in the project include the Champlain Community Care Access Centre (CCAC) and Pembroke Regional Hospital.
