March of Dimes Assistive Devices Program Works to Improve Speed of Service

In Ontario, recent changes in the supports available to people with disabilities along with health care systems under pressure and growth in the number of seniors, have produced a need for faster service from the March of Dimes Assistive Devices Program. In our efforts to meet this community demand we are making changes to our program, processes to improve the speed with which applicants receive funding from our program.

The key changes to our system are as follows:
  • Our Applicant Assessment Form and Equipment Request Form will be combined into one form. The new form has fewer sections and will be accompanied by a User Guide to help applicants complete the form more easily.
  • All required documentation (O.T.’s Assessments, price quotes, Income Tax Notice of Assessments and Ministry of Health’s Responses) will be required at the time of application. This will ensure we have all information needed to grant funding requests in shorter time frames.
  • When assisting applicants in seeking additional funding, our plans and time frames will be clearly communicated to ensure funding decisions are finalized in a timely manner.
  • All written communication documents used by program staff have been revised to ensure they are clear and easy-to-understand

These program changes will be implemented on October 31, 2012. The new application guide and application form will be made available as soon as possible. Older forms will be accepted until December 31, 2012.

No other program changes are being implemented at this time. We are in the process of reviewing and clarifying our eligibility criteria. Any future changes will be communicated as they come into effect.

Please visit our website at www.marchofdimes.ca/adp for more information or to obtain application forms for our program. The new forms will be posted prior to the end of October.
