Primary Care Outreach to Seniors program is expanding!



The South-East Ottawa would like to introduce the expansion of the new Primary Care Outreach to Frail Seniors Program. This Aging At Home initiative is funded by the


Champlain Local Health Integration Network. The program started in 2009 at South East Ottawa CHC and Centretown CHC. It is now being offered at 3 other locations: Somerset West CHC, Carlington CHC and Pinecrest Queensway CHC.

The goal of this project is to work in collaboration with health professionals to reduce isolation and support high risk seniors living at home. This seamless care model will help improve the overall health of these seniors in order to reduce the number of emergency visits and hospital admissions.

Through a shared care approach, nurses are available to visit frail seniors in their home to support you and these clients with their care plan. The community health worker will link them to appropriate community resources and assist them with their various needs.
