The following is excerpted from the inclusion policy for Champlainhealthline.ca.
Champlainhealthline.ca is a comprehensive Information and Referral (I&R) database providing a direct link to local consumer health care and social services information within the Champlain area. The database is a current inventory of available health and social service options to assist consumers to make informed decisions and/or choices. Services are described in a consistent manner to facilitate comparison, and sufficient information is provided on each program or organization to help with the decision process.
Health services are defined as care that enables: "a state of dynamic balance in which an individual's or a group's capacity to cope with all the circumstances of living is at an optimum level." They are representative of the continuum of services across the local health care system - promotive, preventative, curative, rehabilitative, maintenance and palliative.
Social services are defined as such services that address quality of life issues including homelessness, affordable housing, social isolation, unemployment, social assistance, children’s welfare and other basic needs.
The organizations and programs listed on champlainhealthline.ca address the physical, mental and/or social well-being of individuals and populations at every stage of life.
As individual and community health and social service needs change over time, the database will strive to remain responsive to these emerging needs and trends.
Champlainhealthline.ca gives priority inclusion to organizations and programs that:
Are located in the Champlain area;
Provide a direct service to the public;
Provide basic needs services;
Are not-for-profit and/or publicly funded, such as community-based or government organizations, health care facilities, support, advocacy and self –help groups, or commercial organizations subsidized or licensed/regulated by the government or with special contractual agreements to operate long term care facilities and specific home care services;
Provide services that are subsidized, low cost or free;
Accept community self-referrals;
Serve the Champlain region of Ontario but are located outside the community;
Provide services to surrounding areas;
Are involved in licensing, planning or coordinating direct services;
Are commercial organizations that provide services not offered by the not-for-profit sector or are complementary to them;
Are networks or coalitions of direct service providers (if they provide direct service to the public)
Are associations that provide referral services and/or handle complaints about members practice;
Are government or non-government advisory bodies and funding agencies
