Service Description:
|
The major referral centre in Eastern Ontario for complex diabetes care. Referrals are accepted by fax from a referring physician to an endocrinologist. The diabetes team combines experience and expertise with the latest advances in research and education for providing optimal diabetes care. The team members work with patients to create an individualized plan of care based on the their unique health profile and lifestyle.
Individual counselling by a diabetes educator (nurse and/or dietitian) as well as group education classes are available to patients of the centre * classes include two-day type 1 and type 2 diabetes education programs and classes on topics such as flexible insulin therapy and carbohydrate counting. * Referrals to specialists may be made to prevent or delay the progression of complications.
