Service Description: Personal Care/Respite Care: provided by trained Home Support and Personal Support Workers who are employees of OWCS

Adult Day Program: for frail and cognitively impaired seniors * transportation available as resources permit

Home Maintenance/Home Help: includes yard work, odd jobs, housecleaning (heavy and light), snow shoveling

Snow Go Program: low income seniors and people with disabilities can apply to receive financial assistance

Transportation: to medical appointments and OWCS programs * grocery shopping bus, personal shopping bus * extra outings scheduled throughout the year

Foot clinics

Wheels to Meals / Congregate Dining: Luncheon program for frail seniors * includes hot nutritious meal and social activities * Monthly outing, transportation available as resources permit

Telephone Assurance Program (TAP) and Friendly Visiting Programs Aging in Place: specific Ottawa Community Housing Buildings

Supportive housing assisted living services for high risk seniors: by referral only, 24 hour care for high need seniors

Application: Call for more information or to apply. Snow Go Assist program: Participants must provide proof of active enrolment in one or more of the following programs to prove that they meet the program objectives: * GIS (Guaranteed Income Supplement) * ODSP (Ontario Disability Support Program) * Canada Pension Plan Disability Benefit * Ontario Works plus proof of disability and/or senior (60 years and older) * Essential Health and Social Supports plus proof of disability and/or senior (60 years and older) * Home Help plus proof of disability and/or senior (60 years and older) * Low income senior or person with a disability (proof may be required) * Special Circumstances may be considered For more information on the Snow Go program, visit this link: ottawa.ca/en/social_com/seniors/snow_go
Eligibility - Population(s) Served: Seniors, 55 and over, and persons with physical disabilities who wish to maintain themselves in their own homes or apartments and live in the area served * Adult Day Program and Foot Care is open to all City of Ottawa residents * Respite program has an extended catchment area Snow Go Assist program : Seniors and persons with disabilities, must be for the purpose of ensuring mobility * must be physically unable to clear snow from their private driveway and/or walkway due to their own disability * must lack resources (people and/or financial) to arrange for clearing of snow from their private driveway and/or walkway
