Service Description:

Coordinates in-home services for people who are recovering from illness or injury and for those who are living with chronic illness or disability.
In-school services for children with disabilities * placement into long-term care homes * information and referral
In-home services: nursing * personal support * physiotherapy * occupational therapy * speech language therapy * social work * nutritional counselling * medical supplies and equipment
Specialized programs geared to specific health needs: acquired brain injury * convalescent care * child and family services * school health support * mental health * palliative care * respite care

Anyone can make a referral to a CCAC, the client, a family member, caregiver, friend, physician or other health care professional * To make a referral, for more information or to request a referral form, please call.

Individuals who are recovering from illness or injury, and for those who are living with chronic illness or disability * children with disabilities who are in school * must have an Ontario Health Card for home care services
